If you are seeing us for an initial visit, bring in your completed Medical Information Forms. Please follow Steps 1 thru 8 below to retrieve the forms.

1) If Adobe Acrobat is installed on your computer proceed to Step 3. If not or "you don't know" continue to Step 2.

2) Click on the Adobe icon below and follow the instructions to download Adobe Acrobat.

3) Click "Medical Information Form" below to download the form to your computer. Please print the form.

4) Complete the Medical Information Form and bring it with you to your first appointment.

5) Click on "HIPPA" below to download the notice, then print the form.
